Step 2
Fill out the paperwork.
There are two ways to fill out your paperwork:
- Use Adobe Acrobat Reader to download, fill out and save your paperwork.
- Use Google Chrome or Microsoft Edge web browsers to fill in PDFs.
- Open the new patient paperwork link in Chrome or Edge.
- Click into the fields and type your information.
- In Google Chrome, you have to print the edited form as a PDF. After filling in the form, click the print icon in the top right corner of the page. For Destination, select Microsoft Print to PDF. Click Print. Specify the location in which to save the file. Click Save.
- In Microsoft Edge, simply click the Save icon in the top right corner of the page after filling in the form.
